Surviving are his brother, J.T. Cook, Burlington, NC; sisters, Kayla Anthony, Ash, NC, and Laurin Anthony, Ash, NC; niece, Emberlee; nephew, Austin; paternal grandmother, Brenda Hughes, Ocean Isle Beach, NC
